Conditions for Consideration of Funding:

Please note that all conditions must be acknowledged and adhered to for the Foundation to consider supporting the organization.

If any or all of the gift is designated towards an event sponsorship, please note that a blessing is required at the beginning of the event program and/or dinner. More details will be provided upon funding approval.

As a condition of funding at the presenting/underwriting sponsor level, I agree to use Steward Family Foundation preferred vendors.

Please note that as a condition of funding, it is required for all grantees to submit yearly reports to the Steward Family Foundation.

Public recognition of the Foundation(s), including but not limited to use of logo and disclosure of the award amount, is contingent on explicit written approval on a case by case basis.

I am aware that the completion of this application does not guarantee funding and that the amount requested may differ from the actual award.

I understand that funding disbursement cycles may vary.
